Call for Health Requirements for Financial Assistance

The patient or immediate family member (spouse or child) must have worked as an agricultural worker within the last two years or be a retired or disabled worker. Individuals not meeting the six month criteria are reviewed on a case by case basis.

All other local and national resources must have been explored and exhausted.

Financial Assistance is provided for:

  • Specialty Care
    A referral made by the clinic's physician to an out-of-clinic specialist
  • Diagnostics
    Lab work, x-rays, CT scan, MRI
  • Surgeries
    Must be considered critical by physician
  • Vision
    Eye glasses needed for work purposes
    Surgeries needed to prevent loss of vision
  • Dental
    Oral surgeries due to infection
    Additional assistance on a case-by-case basis
  • Medications
    $100 maximum

*Additional assistance for other medications on a case-by-case basis

**If possible, a $10 co-payment may be required from the patient. **

Call for Health will NOT:

  • Reimburse less than $20 service fees. (Due to administrative costs)
  • Pay for cosmetic treatment.
