Performance Management and Governance Tool Kit

Over the years, community & migrant health center staff have articulated a need for sample practice management materials, including forms, policies, procedures, etc. that can be adapted in service delivery for their migratory and seasonal agricultural worker (farmworker) users.  In response, the National Center for Farmworker Health has created a Performance Management and Governance Tool Kit which provides Health Center Program grantees with sample policies, procedures, forms, and other supportive documents specifically designed according to administrative and governance program expectations.

To view the list of program expectations for all entities funded by the Bureau of Primary Health Care (BPHC) under section 330 of the Public Health Service Act, as amended by the Health Centers Consolidation Act of 1996, please click here.

In addition, health centers are faced with a list of program requirements mandated to them by the Bureau of Primary Health Care.  These program requirements fall under the general headings of Need, Services, Management and Finance, and Governance. One of the goals of this tool kit is to assist health centers in meeting these requirements, and many of the tool kit sections address the requirements directly.

This tool kit is a work in progress; NCFH will periodically add other topics and tools in response to health centers’ requests.  We would like to express our gratitude to the health centers that shared their policies, procedures, and forms with us and allowed us to make these available to you as part of the tool kit.

We hope that you find this tool useful and we invite you to send us your comments and share your administration and governance best practices with us.  If there are other topics you would like to see included in this tool box, please contact Hilda Bogue at or (512) 312-2700.

The tool kit is divided into several sections.  Under each of these sections are various topics which contain sample forms, policies, and procedures.   Click the links under each section heading below to access specific pages.

Note: The terms Agricultural workers and Farmworkers are used interchangeably within the tool box.
