Performance Management and Governance Tool Kit

Over the years, migrant health center staff have articulated a need for sample practice management materials, including forms, policies, procedures, etc. that can be adapted in service delivery for their migrant and seasonal farmworker users.  In response, the National Center for Farmworker Health has created a Performance Management and Governance Tool Kit which provides Migrant Health grantees with sample policies, procedures, forms, and other supportive documents specifically designed according to the administrative and governance program expectations.

 In addition, health centers are faced with a list of program requirements mandated to them by the Bureau of Primary Health Care.  These program requirements fall under the general headings of Need, Services, Management and Finance, and Governance. One of the goals of this tool kit is to assist health centers in meeting these requirements and many of the tool kit sections address the requirements directly.  

 This tool kit is a work in progress and NCFH will be periodically adding other topics and tools to respond to health centers’ requests.   We would like to express our gratitude to the health centers that shared their policies, procedures, and forms with us and allowed us to make these available to you.
