Through our partnerships, we have gathered a few key resources and tools below to support this initiative to prevent, manage, and treat prediabetes and diabetes.

• Farmworker Nutrition Messaging Tips & Social Media Resource: Nutrition plays a critical role in the health and well-being of farmworker communities. How we talk about nutrition matters just as much as the food itself. NCFH’s Farmworker Nutrition Messaging Tips resource offers guidance and best practices on taking a culturally inclusive and practical approach when talking about nutrition with farmworkers. Accompanying the best practices resource, find ready-to-use social media materials developed by NCFH focused on portion control and healthy eating. These materials include social media graphics, video, and messaging in English and Spanish. 
• Making a Case: Food is Medicine in Primary Care Settings: Food is Medicine (FIM) programs have gained attention in recent years as an effective intervention for nutrition in the efforts to improve chronic conditions, and health centers are a crucial part of those efforts. Learn why it matters and the benefits for patients, local economies, and health centers.
• Do Not Let Diabetes Harm Your Kidneys: This patient flyer helps health center staff teach patients more about how diabetes and kidney health are connected. • Heeding the Signs of Prediabetes. Radio broadcast was done in partnership with ADCES and Radio Bilingue on prediabetes. Originally broadcast June 15, 2022. Health educators discuss the ABC of prediabetes, the Do’s and Don’ts, the ways to make healthy lifestyle changes, and the National Diabetes Prevention Program. • Guide for Patient Management of Chronic Illness through “Tele-Education”: This Guide for Patient Self-Management of Chronic Illness, along with the attached Participant Telehealth Readiness Tool (Appendix A), provides Health Centers a method of assessing a participant’s readiness for tele-education and describes how to use messaging services to increase and drive patient self-management of chronic illnesses like diabetes, high blood pressure, cholesterol, and others.
